Responsibilities:

Position Summary:

This position provides comprehensive management and coordination of care for patients requiring Kidney and Pancreas Transplantation. The level and intensity of care coordination is situational and appropriate based on patient need. The areas of transplantation covered are: pre-transplant evaluation, wait list management, post transplant follow up and the timely placement of organs.

Qualifications:

THIS POSITION WILL BE FILLED AS EITHER A TRANSPLANT COORDINATOR I OR TRANSPLANT COORDINATOR II, DEPENDING ON QUALIFICATIONS. 

Transplant Coordinator I

Required Qualifications:

  • Working knowledge of CMS/UNOS requirements
  • Must possess the skill, knowledge, and ability essential to the successful performance of assigned duties
  • Must have a minimum of 1 year experience caring for transplant or outpatient dialysis patients within the last 3 years (ex. dedicated transplant unit, ambulatory transplant care, transplant coordinator role, outpatient dialysis center)
  • Must demonstrate customer service skills appropriate to the job
  • Minimum 2 years Critical Care, or Step Down, or Dialysis, or the Transplant setting
  • Excellent written and verbal communication skills in English
  • Demonstrated knowledge of immunosuppressive therapies and care of the post-transplant patient
  • Ability to maintain a work pace appropriate to the workload
  • Ability to establish and maintain effective working relationships across the organization


Preferred Qualifications:

  • Previous transplant or organ procurement experience preferred


Licenses and Certifications:

  • Registered Nurse
  • Basic Life Support


Education:

  • Bachelor of Science, Nursing (Required)
  • MASTER OF SCIENCE IN NURSING (Preferred)

Transplant Coordinator II

Required Qualifications:

  • Working knowledge of CMS/UNOS requirements
  • CCTN or CCTC or CPTC certification required
  • Must possess the skill, knowledge, and ability essential to the successful performance of assigned duties
  • Must demonstrate customer service skills appropriate to the job
  • Minimum of 3 years Transplant experience
  • Excellent written and verbal communication skills in English
  • Demonstrated knowledge of immunosuppressive therapies and care of the post-transplant patient
  • Ability to maintain a work pace appropriate to the workload
  • Ability to establish and maintain effective working relationships across the organization


Preferred Qualifications:
